    Admitted to No 1 NZ Field Ambulance, 22 February 1918 (pyrexia, unknown origin); transferred to 4th Division Rest Station, 23 February 1918; to 14th General Hospital, Wimereux, 28 February 1918; discharged to duty, 5 April 1918; rejoined 1st Bn, Canterbury Infantry Regiment, in the field, 12 April 1918.
    Admitted to No 3 NZ Field Ambulance, 15 May 1918; transferred to 29th Casualty Clearing Station, 21 May 1918 (trench feet); to 1st Australian General Hospital, Rouen, 24 May 1918; to England, 27 May 1918, and admitted to No 1 NZ General Hospital, Brockenhurst, 28 May 1918.
    Classified unfit for Active Service by Medical Board, 30 September 1918.
    Commenced return to New Zealand from Liverpool on board HT 'Maunganui', 2 December 1918; discharged (no longer physically fit for War Service on account of illness contracted on Active Service), 14 March 1919.
