    • David West was the son of Ernst Vilhelm West and Carrie Buick West of Palmerston North, Wellington. His brother was Squadron Leader Peter Wynn Mason West, DFC who also died on active service. Source: Martyn, For Your Tomorrow, Vol.3, p.501.

    • David was training to fly Westland Lysanders (a large STOL aircraft) into makeshift air strips in France. The cargo would be French Underground personnel and supplies. His log book entry completed by the Commanding Officer of the unit records the training accident that claimed the lives of David and a fellow pilot, they were in a Miles Magister.
