    He died between 2 July and 8 July 1943.

    Donald McInnes's portrait published in the Auckland Weekly News, 18 August 1943 incorrectly records a DSM, this is incorrect. After a review, July 2015, of the service file his medal entitlement has been corrected. The Researcher, NZDF Medals Entitlement advised: "There is NO record of the issue of the DSM (Distinguished Service Medal) on his service file. The DSM was awarded for acts of bravery in the face of the enemy for senior and junior ratings of the Navy, there is no evidence on his file that he served in the Navy so it is unlikely that he would have been awarded this medal." CWGC have been advised and the Cenotaph record corrected 3 August 2015. AWMM
