Chronicles Of The N.Z.E.F. - Records of Matters Concerning the Troops, No 44., May 24th, 1918 at page 173 records the following commentary on Cook:

Lieut.-Colonel C.T.D. Cook, D.S.O., was an M.A. and LL.B. of Canterbury College. He was the son of the Late Professor Cook, of Christchurch, and was born in 1883. Before the war he was practicing with the firm of Fullerton-Smith, Miles and Cook, barristers and solicitors, of Marton. He joined up with the main body, and left as captain, second in command of the Hawkes Bay Company of the Wellington Infantry. In Egypt he was attached to Brigade H.Q., and acted as Staff-Captain to the Infantry Brigade on Gallipoli until the end of September, 1915, when he evacuated sick to England. He rejoined his regiment early in January, 1916, and assumed command of the Hakes Bay Company. He was promoted Major 1st March, and appointed second in command of the battalion. He received his Lieut.-Colonelcy on the 15th March, 1917, and given command of the 1st Battalion of his regiment; which he retained until he was evacuated sick from France barely a month before his death. He was mentioned in despatches - Gallipoli - and received the D.S.O. for work in France.

The Colonel was a gentleman in every sense of the word, and would not thank anyone for eulogising him; although one cannot do otherwise but give him great credit for his qualities. By his death the Wellington Regiment has suffered a severe loss, and his memory will long be reverenced by those who were fortunate enough to be under his command.

He died of spinal meningitis at Netley Hospital, on the 2nd May, and was buried at Reading on Saturday, May 4th, with full military honours. The pall-bearers, the firing party and the gun-carriage were all from the New Zealand Force.

Six months before his death he married Miss Haynes, of the town where he is now buried." AWMM
