    • Proceedings of the Court of Enquiry assembled at Armentieres 14 June 1916 by the order of the G.O.C. NZ Division for the purpose of enquiring into the cause of the bomb explosion which occurred at the Div Grenade School June 13th 1916. The court was of the opinion that the accident was caused by Sergt Barber using a live No 20 Vaucluse Grenade for instructional purposes, contrary to orders and used same in such as manner as to explode the grenade whereby the casualties occurred.
