    Wounded in action, 27 September 1916 (gun shot wound, back); admitted to No 3 Stationary Hospital, Rouen, 28 September 1916; transferred to England, 29 September 1916, and admitted to 3rd Southern General Hospital, Oxford, 30 September 1916.
    Died of wounds (gun shot wound, spine; paraplexia; heart failure), 14 October 1916.
