    Admitted to No 2 Australian Casualty Clearing Station, 3 August 1916 (lachrymal infection); transferred to No 13 Stationary Hospital, Boulogne, 4 August 1916; discharged to NZ Base Depot, Etaples, 23 August 1916; rejoined unit, in the field, 26 August 1916. Found guilty, 20 August 1916, of being absent without leave from 9 pm to 5 am, 20 August 1916: awarded forfeiture of 3 days' pay.
    Reported missing, in the field, 21 September 1916; admitted to No 1 NZ Field Ambulance, 22 September 1916 (gun shot wound, right thigh); transferred to 20th General Hospital, Camiers, 24 September 1916; to England, 2 October 1916, and admitted to 1st Southern General Hospital, Birmingham, 3 October 1916. Transferred to NZ Convalescent Hospital, Hornchurch, 4 December 1916; discharged to NZ Command Depot, Codford, 5 December 1916.
    Declared by Court of Enquiry assembled at NZ Command Depot, 16-18 January 1917, to be absent without leave for over 21 days.
    Discharged from Venereal Section, 18 May 1917.
    Found guilty by District Court Martial, Codford, of being absent without leave when under orders to report at NZ Command Depot on 26 December 1916 until 9 pm, 18 January 1917: awarded 40 days' detention, and forfeited 24 days' pay; forfeited 140 days' gross pay.
    Admitted to No 3 NZ General Hospital, Codford, 12 July 1917 (scabies); attached to NZ Command Depot, Codford, 24 July 1917.
