    Admitted to hospital, 30 September 1916; admitted to No 24 General Hospital, Etaples, 7 October 1916 (dysentery, suspected typhoid); transferred to isolation, 9 October 1916; transferred from isolation, No 24 General Hospital, to No 6 Convalescent Depot, 26 October 1916 (colitis); to No 5 Convalescent Camp, Etaples, 31 October 1916; to NZ Base Depot, Etaples, 12 November 1916.
    Admitted to No 24 General Hospital, 23 November 1916 (asthma, acute bronchitis).
    Died of acute bronchitis, No 24 General Hospital, Etaples, 10 December 1916.
