    Admitted to No 1 NZ Field Ambulance, 18 August 1917 (myalgia); transferred to No 2 Australian Casualty Clearing station, 28 August 1917; to No 16 General Hospital, Le Treport, 30 August 1917; to No 3 Convalescent Depot, Le Treport, 6 September 1917; to NZ Base Depot, Etaples, 28 September 1917; rejoined Bn, in the field, 27 October 1917.
    Admitted to 1/1 North Riding Field Ambulance, 12 December 12917 (myalgia); transferred to 3rd Australian General Hospital, Wimereux, 14 December 1917; to England, 19 December 1917, and admitted to Military Hospital, Endell Street, London, 20 December 1917; transferred to NZ Convalescent Hospital, Hornchurch, 29 December 1917.
    Classified 'unfit' by Medical Board and placed on NZ Roll, 15 March 1917.
