    Admitted to 1/1 North Riding Field Ambulance, 12 December 12917 (myalgia); transferred to 3rd Australian General Hospital, Wimereux, 14 December 1917; to England, 19 December 1917, and admitted to Military Hospital, Endell Street, London, 20 December 1917; transferred to NZ Convalescent Hospital, Hornchurch, 29 December 1917.
