    Appointed Company Cook, 14 March 1918; relinquished appointment as Company Cook, 18 March 1918.
    Appointed Company Cook, 20 April 1918.
    On leave to United Kingdom, 16 October 1918; rejoined unit from leave, 2 November 1918.
