    Attended St John's College (1905-10).

    Brother of Gunner Hugh Douglas Forde (13505), who died of wounds on 26 October 1917, in France.

    Embarked Wellington, 12 February 1917; disembarked Plymouth, England, 2 May 1917, and marched in to Sling Camp.
    Proceeded overseas to France, 6 June 1917; joined 1st Bn, Wellington Infantry Regiment, in the field, 26 June 1917.
    Admitted to No 3 NZ Field Ambulance, 24 July 1917 (sprained left ankle: accidental injury, soldier in no way to blame; 'the disability is of a trivial nature'); discharged to duty and rejoined unit, 3 August 1917.
    Killed in action, Belgium, 3 October 1917.
