    Admitted to No 37 Casualty Clearing Station, 25 May 1916 (pleurisy); transferred to No 12 Stationary Hospital, 3 June 1916 (bronchitis); to No 1 Australian General Hospital, Rouen, 11 June 1916; embarked for England, 19 June 1916; admitted to No 3 London General Hospital, Wandsworth, 22 June 1916; transferred to No 4 London General Hospital, 25 July 1916; left NZ Convalescent Hospital, Hornchurch, 2 August 1916 (tuberculous).
